2021 CMPA MLR payments now processing

The Alberta Medical Liability Reimbursement (MLR) Program has begun processing reimbursement payments to physicians for 2021 CMPA fees. For those participants registered for Early Reimbursement your payments should have been deposited on April 30. For those participants that are reimbursed quarterly, payments will be deposited on May 7.

For a record of your reimbursement payment, consult your Alberta Health Statement of Account and observe the line item "Med Liab Reimbursement Pgm."

If personal information has changed (e.g. identifying information, bank information, etc.), physicians must submit a new application with the updated information. Failure to notify the Alberta MLR Program of changes may result in non-payment of reimbursement.

For more information about the Alberta MLR Program, visit the official website or see General Bulletin 130. For enquiries related to this message, or to the MLR program in general, email Health.MLRProgram@gov.ab.ca.

Using anticoagulants among older adults with recurrent falls

Case
Mrs. Green is a mildly frail, 85-year-old who had a ground-level fall. This is her second fall in 12 months. She reports her heart is racing. Her blood pressure is 135/70 without orthostatic changes. An ECG shows new atrial fibrillation (AF) with a heart rate of 120, and a beta-blocker is initiated for rate control.

Background
The prevalence of nonvalvular AF increases significantly with age, from 0.7% in adults under age 60 to 17.8% in those over 85. Ischemic stroke and systemic embolism are the major adverse consequences. Because the annual stroke risk increases up to 4.4% in those over age 75, starting therapy for stroke prevention is the standard of care. However, all anticoagulants and antiplatelet agents increase the bleed risk, with intracranial hemorrhages being most concerning.

Issue
Upwards of 30% of Canadians with AF who would otherwise qualify for anticoagulation do not receive therapy due to concerns of frailty and falls. What is some of the evidence regarding using anticoagulants among older adults with recurrent falls?

Thank you for your understanding.

Are You a Wolf EMR User?

There are already 189 physicians in 40 clinics using Wolf that are live on the Community Information Integration/Central Patient Attachment Registry (CII/CPAR), and another 108 physicians in 23 clinics in progress to implement and connect their clinic EMR to the rest of the health system.

Recognizing that Wolf will be retired in December 2023, some providers are still choosing to join CII/CPAR now to take advantage of the benefits to themselves and their patients. TELUS has committed to a smooth transition and clinics will not need to sign up twice for CII/CPAR.

Dr. Joseph Ojedokun, who has a family practice in Whitecourt, says, “studies have shown that continuity of care is the single most important intervention that when offered to patients through timely access, can reduce mortality by up to 50%. As enablers of continuity of care, CII/CPAR and eNotifications are in my opinion, the best things that have happened to Primary Care in Alberta”.

This article is the second in a series to highlight CII/CPAR implementation experience with the five conformed EMRs. Watch future issues for a focused look at each EMR.

Survey Corner

Health care partners and other organizations often seek physician feedback through surveys. This month, the following is available for your consideration:

Physician Burnout Survey
This study aims to evaluate the impact of burnout on medical students, residents and physicians in Alberta, and identify the individual, occupational organizational and COVID-19 pandemic related factors that influence burnout. Survey conducted by University of Alberta, Faculty of Medicine and Dentistry, Department of Psychiatry. Approved by research ethics board. Ethics ID: Pro00091436.

Motivational Communication competency assessment test (MC-CAT)
This project intends to validate a new brief (15 minute) user-friendly, interactive online competency assessment tool to assess health care providers’ use of a Motivational Communication approach with their patients to support lasting health behavioral changes in their patients suffering from chronic diseases. Approved by the Research Ethics committee of the CIUSSS du Nord-de-l’Île-de-Montréal.

PAUSE App Co-Design Study: Helping patients to better understand medication sick day guidance
Research teams at the Universities of Alberta and Calgary are conducting a study to explore the use of an eHealth application that provides medication sick day guidance to acutely ill patients with chronic conditions. Primary care physicians are invited to one 40-minute or two 20-minute virtual interviews to share about barriers and facilitators they face in delivering this information to patients. A $100.00 honorarium will be offered for their time.
